submit a glitch form!

forgot to put this up earlier, so im doin it now..

wildirish mentioned we should have a form where you can submit your glitches, so i decided to do it.. on the portal theres a little form towards the bottom right

now im hoping this doesnt get out of hand and i become spammed with shit, but im sure it wont =]
